Prostaglandins play a pivotal role in the regulation of the pulmonary vascular tone. Inhibition of cyclooxigenase using aspirin or indometacin enhances hypoxic pulmonary vasoconstriction. In oleid acid- or micobead- (50-150 µm) induced acute respiratory distress syndrome cyclooxigenase inhibitors improve gas exchange by increasing pulmonary arteriolar resistance in non-ventilated areas (Leeman et al., 1992, Naeije et al., 1987). Thus, although NSAR inhibit platelet aggregation, they should not be given to patients with pulmonary hypertension because of their effect on pulmonary vascular resistance. Furthermore, it is a physiological nonsense to treat pulmonary hypertension with prostanoids and simultaneously give cyclooxigenase inhibitors to the same patient.
