Betablockers

Beta-receptors are involved in the regulation of the pulmonary vascular tone and myocardial contractility. In acute hypoxia, beta-blockers increase pulmonary vascular tone (Brimioulle et al., 1997). In patients with severe mitral stenosis, beta-blockade increases pulmonary vascular resistance despite a decrease in left atrial pressure, cardiac output and systemic blood pressure (Wisenbaugh et al., 1993). Thus, beta-blockers have not only a negative inotropic effect on the myocardium, but they may also impair coronary perfusion pressure by lowering the systemic blood pressure. Therefore, beta-blockers should be avoided in pulmonary hypertension.
