HIV therapy


Pulmonary hypertension in patients with HIV infection affects the prognosis of these patients. In a Swiss cohort study, the median survival was 1.3 years in patients with pulmonary hypertension and 2.3 years in a matched control group (Opravil et al., 1997). Antiretroviral treatments affected significantly the development of pulmonary artery pressure. During a median follow-up of 1.3 years, pulmonary artery pressure increased by 19 mmHg in untreated patients, whereas it remained unchanged in those with an antiretroviral treatment (Opravil et al., 1997). Moreover, a recent cohort study on 1042 patients has shown that the incidence of HIV infection associated involvement of the pulmonary circulation and the heart decreased from 52 % to 19 % after the introduction of highly active antiretroviral therapy (Pugliese et al., 2000). Thus, there is indirect evidence that treatment of HIV infection improves pulmonary vascular resistance, and hence the prognosis of these patients.
