Angiography

Pulmonary angiography continues to be the gold standard for the evaluation of surgical candidacy in patients with CTEPH (6). It should always be performed in addition to helical CT if there are any perfusion defects on the V/Q scan, even if they are small. The angiography should only be performed in centers performing pulmonary thrombendarterectomy in close cooperation with the surgical team.

The characteristic appearance of CTEPH includes abrupt cut-offs or narrowing of major pulmonary arteries, vascular webs or band-like constrictions, pouching defects and intimal irregularities with marked tortuosity of the patent vessels suggestive of organized thrombus.
